Epidermolytic acanthoma
Epidermolytic acanthoma (pronunciation: epi-dermo-lytic a-can-thoma) is a rare, benign skin lesion that is characterized by hyperkeratosis and acantholysis.
Etymology
The term "epidermolytic" is derived from the Greek words "epidermis" (the outer layer of the skin) and "lysis" (breakdown or destruction), while "acanthoma" comes from the Greek words "akantha" (thorn or spine) and "-oma" (tumor). Thus, "epidermolytic acanthoma" refers to a skin lesion that involves the breakdown of the epidermis and the formation of a thorn-like tumor.
Description
Epidermolytic acanthoma typically presents as a solitary, small, well-demarcated papule that is often skin-colored or slightly hyperpigmented. It is most commonly found on the trunk, genitalia, and extremities. The lesion is usually asymptomatic, but it may occasionally cause mild itching or discomfort.
Histology
Histologically, epidermolytic acanthoma is characterized by hyperkeratosis, acanthosis, and papillomatosis. The most distinctive feature is the presence of epidermolytic hyperkeratosis, which is a histologic pattern of keratinocyte degeneration characterized by vacuolar alteration of the granular layer, perinuclear vacuolization of the upper spinous layer, and coarse keratohyalin granules.
Diagnosis
The diagnosis of epidermolytic acanthoma is typically made based on the clinical presentation and histologic findings. A skin biopsy may be performed to confirm the diagnosis and rule out other conditions, such as seborrheic keratosis, verruca vulgaris, and condyloma acuminatum.
Treatment
Treatment of epidermolytic acanthoma is usually not necessary, as the lesion is benign and often asymptomatic. However, if the lesion is bothersome or cosmetically unacceptable, it may be removed by methods such as cryotherapy, electrocautery, or surgical excision.
